This Privacy Policy explains how Direct Primary Care Associates, PLLC (“DPCA,” “we,” “us” or “our”) collects, uses and protects information when you visit directprimarycareassociates.com and any web address that redirects to it (the “Site”).
This policy covers the Site only. Health information you share with us as a patient is protected under our Notice of Privacy Practices, which describes how DPCA uses and discloses protected health information and your rights regarding it.
Information we collect
Information you give us. When you complete a form on the Site, we collect what you enter, typically your name, email address, telephone number, organization or employer, and your message. Our forms do not ask for health information and instruct you not to include it. If you enter health information in a message field, we will use it only to respond to your inquiry and will remove it from our website and marketing records once your inquiry is handled.
Information collected automatically. We use privacy-focused analytics that does not use cookies, does not use persistent identifiers and does not track you across other websites. It records aggregate, non-identifying information such as the pages viewed, the general region a visit comes from, the type of device and browser used, and how visitors arrive at the Site. How we use information
We use the information we collect to:
- respond to your inquiry and provide information you request about membership, employer plans or our clinics;
- contact you by phone, text or email about your inquiry;
- operate, maintain, secure and improve the Site; and
- comply with law and protect the rights, property and safety of DPCA, our patients and others.
How we share information
We do not sell personal information. We do not share personal information with third parties for their own marketing.
We share information only with service providers that help us operate the Site and respond to inquiries, and only as needed for that purpose. These providers are required to protect the information and may not use it for any other purpose. We may also disclose information when required by law, subpoena or court order, or when necessary to protect the rights, property or safety of DPCA, our patients or others.
Text messages
If you provide a mobile number, you agree that DPCA may send you text messages about your inquiry. Message frequency varies. Message and data rates may apply. Reply STOP to opt out at any time or HELP for help. We do not sell, rent or share mobile numbers or text messaging consent with third parties for their marketing.
You may opt out of marketing email at any time using the unsubscribe link in any message. We may still send messages related to an inquiry you made or, if you are a member, your membership.
Links to other websites
The Site may link to websites DPCA does not operate. This policy does not apply to those websites, and DPCA is not responsible for their content or privacy practices.
Your choices and requests
You may ask us to access, correct or delete personal information you submitted through the Site by contacting us using the details below. We will respond within a reasonable time and may need to verify your identity before acting on a request. Requests about your medical records are handled under our Notice of Privacy Practices.
Security
We use reasonable administrative, technical and physical safeguards to protect information submitted through the Site. No method of transmission over the internet or electronic storage is completely secure, and we cannot guarantee absolute security.
Children
The Site is not directed to children under 13, and we do not knowingly collect personal information online from children under 13. If you believe a child has submitted personal information through the Site, contact us and we will delete it.
Changes to this policy
We may update this policy from time to time. The effective date at the top shows when it was last revised. Continued use of the Site after a change means you accept the revised policy.
